A Revocation of Financial Durable Power of Attorney is an instrument revoking the authorization for a person to act on the behalf of another, should he or she become incapacitated due to an accident or illness, limited to financial matters.  In many instances, a person may have more than one power of attorney, so it is important that you are very specific in identifying the specific power of attorney you want to revoke.  For example, a person may have two Durable Powers of Attorney: one for financial matters, and the other for health care decisions.  If you execute a Revocation of Financial Durable Power of Attorney, you are cancelling the appointment of an agent to act on your behalf in all financial matters a principal would be capable of doing if he/she were not incapacitated.

If you have granted a Financial Durable Power of Attorney to a person who is acting as your agent, but now wish to revoke such power, you may use this form.  If you want to draft another Financial Durable Power of Attorney appointing a different agent or if you have not created a Financial Durable Power of Attorney, you can use this Financial Durable Power of Attorney.
