This is an example of a completed Power of
Attorney for Financial Decisions, to allow another person to take care of the
financial matters of a person who has become mentally incapacitated, either
through accident, illness or other means. The Power of Attorney gives
the Agent wide latitude in dealing with the Principal's financial decisions, so
that the Agent can adequately manage the Principal's affairs, without the need
to have a conservatorship established. Any power which the Principal does
not want to convey can be crossed out and initialed by the Principal.
The Power of Attorney automatically is
revoked in the event the Principal regains mental capacity.
